Insurance Claim Module Draft Proposal

I have been working with @Saransh Sharma <sa...@muellners.com>  on
insurance claim module and have come up with a draft.
I would really appreciate if people in the community are willing to have a
look at it also suggests things which I could add or remove from my sample
design.
