XML, XSL and HTML Forms

I was wondering what people are doing when it comes to putting up forms in
their cocoon sites. Is there an accepted set of tags for representing
forms in XML that is reasonable for translation to...for instance...HTML
and WML?

I have found plenty of examples of generic XML that is turned into
HTML/WML however I have yet to find an example of a form.

Any suggestions for where I might go for additional information?
