ErrorDocument use for mod_proxy

We are trying to have mod_proxy produce nicer error results, but all
the error messages in the proxy modules return HTTP_INTERNAL_SERVER_ERROR.

So, it is kosher to define additional error codes:
	HTTP_PROXY_HOST_NOT_FOUND	520
	HTTP_PROXY_OTHER_ERROR		521
	
Given unique error codes, it looks like ErrorDocument will do what is needed.
