Allow number of fields to be supplied when field names are not known in DBOutputFormat#setOutput()

DBOutputFormat expects the column names to be included in the query. However, if the field names are not available, only the number of fields is sufficient for preparing the SELECT query. 

This issue will add API support for supplying the number of fields in DBOutputFormat#setOutput().
