NoTargetForURIException

hi i am configured the all details but still this error is coming in form
page action tag  url name and struts-config.xml page   path are the same and
giving appropriate  

   Error 404:
com.ibm.ws.webcontainer.servlet.exception.NoTargetForURIException: No target
servlet configured for uri: /ujjivanInsurance
