   Fixes #2353
   
   ## Summary
   - change persisted Topic and Consumer Group identity from `(cluster_id, name)` to `(cluster_id, instance_id, name)`
   - scope Apache metadata reads, writes, deletes, and instance resource counts by persisted `instance_id`
   - add an idempotent MySQL upgrade script for existing deployments
   
   ## Migration / dedupe policy
   - normalize legacy `NULL instance_id` rows to the explicit legacy scope `''`
   - before adding the new unique key, deduplicate exact `(cluster_id, instance_id, name)` collisions by keeping the most recently modified row
   - when `gmt_modified` ties, keep the row with the larger `id`
   
   ## Verification
   - `JAVA_HOME=$(/usr/libexec/java_home -v 21) mvn -Dtest=RocketMQAdminClientImplTest,RocketMQMetadataProviderTest,ApacheInstanceProviderTest,MybatisPlusInstanceRepositoryTest test`
   - `git diff --check`
