PatternLayout manual page does not mention that this layout supports headers and footers.

* charset
* pattern
* replace
* alwaysWriteExceptions

Undocumented parameters:
* config
* noConsoleNoAnsi
* header
* footer

I'm thinking to only document the {{header}} and {{footer}} parameters, but  should the others be documented also?
