[kafka] Separate value decoding and preserve record metadata

Use the shared table contract for binary and strict UTF-8 string value
decoders. Preserve the partition-aware transcoder API and pass each full
record to row assembly so optional key, headers and three-field metadata
retain their input semantics. Binary values keep borrowed array storage.

Cover invalid UTF-8 and nullability failures with declared metadata:
failed partitions append no prefix, and reused plans preserve missing
timestamps, batch types and the next request's partition after failure.
