CustomLog documentation

The CustomLog directive is documented twice, once with the env=X option,
and once without. Wouldn't it be better to document it once, with
env=X marked as a optional argument?

That's the way I did it originally, but it looked really
confusing and ugly.  CustomLog has two different levels
of enhancement now: format nicknames and conditional
expressions.  I couldn't figure out a way to make
the docco clear without separating the two -- if you can
manage it, brilliant.
