multiple instances of same form?

Hi,

Is it possible to have multiple instances of the same form on one JSP?
If yes,
how can you make sure that only one form gets submitted at a time?

In the United States Form Names have to be different or at least 
be referenced by different indix value as in document.forms[index]
could you provide example of 2 forms with same appelation?
