Added rules-all debug channel

All:

I had to add some more logging to be able to locate the runaway rule to 
help out Piotr today. It's under the "rules-all" channel and is here if 
anyone wants to review:

This should help troubleshoot similar situations in the future. Wiki is 
updated, too.
