Problem with Can't Find Prefix

I am having a problem with attachments.  So to test a theory I decided to 
try and get the samples/attachment application running.

When I register the service and then attempt to generate the WSDL I get the 
error:

Can't find prefix for 'urn:EchoAttachmentsService'.  Namespace prefixes 
must be set ...

I did a search on the axis mailing lists and it seems this question has 
been popping up recently, but I cannot seem to find a response containing 
an answer.

Could somebody please help what is going wrong?
