Umm, does anyone read the instructions that are there in the commit
template?

Obtained from: is supposed to be used in cases where you "stole" code
from somewhere without the author actually submitting it to the project
e.g. you spot something nice in NCSA and bring the code into Apache.

Submitted by: is supposed to be used when someone *submits* code to us

Reviewed by: should be used to indicate that someone other than the
commiter reviewed the code.

So the above should have just had
