ManifoldCF on trunk now has pipeline support! (Warning: SCHEMA CHANGE)

I've merged branches/CONNECTORS-946 to trunk.  Not only were pipelines
added, but also a number of other bugs were fixed that the pipeline rework
uncovered.

Please be warned: this represents a schema change from MCF 1.6.x.  The
upgrade code has been implemented, but not yet tested!  Any problems you
encounter, please let me know.
