In the Table of Contents, the TOC itself is listed as item/chapter 1. However, in the document itself the first item/chapter of the pdf.xml is listed as item/chapter 1, whereas the TOC lists it as item/chapter 2. This is confusing...

The attachment is the well-known pdf generated from the filtering testcase of the maven-pdf-plugin, version 1.3-SNAPSHOT. It shows on page "i" the Table of Contents having a self-reference to itself as item/chapter 1. In the TOC, the chapter "Introduction" is defined as item/chapter 2 on page 1. Having a look at page 1, the item/chapter "Introduction" is labelled as item/chapter 1.
