1.4 -> 1.6 upgrade instructions

Google wasn’t helpful, so:

I have a cluster (Hadoop 2.2, HBase 0.96) managed by Ambari 1.4

I want to upgrade to Hadoop 2.4, HBase 0.98 managed by Ambari 1.6 (or later)

Are there instructions for performing this upgrade  (both the HDP and Ambari pieces) somewhere?
