Unable to create client in Chrome browser

Navigate to client page 
Click on +Client button, in Create Client page 
1) External id text box
2) Active check box
3) Submitted on Calendar
4) Open Savings account check box 
Above mentioned fields are not visible. regularly observed in chrome and randomly noticed in Firefox browsers
