landscape.pdf is probably a better example to work from.  Created using Acrobat and changed the Page Setup to landscape.

orientation-acrobat-landscape.pdf was created the same way, except an existing simple portrait pdf was inserted at the end.  When debugging the document, it seems to struggle with  the order of the media box coordinates and upon manual inspection, there are multiple declarations of object 26 (the first page).  The first shows /MediaBox[0.0 0.0 612.0 792.0] and the second shows /MediaBox[0.0 0.0 792.0 612.0].
