For me refreshing doesn't help, the checkbox still doesn't show up.

CouchDB version = Apache CouchDB 1.5.0 on Linux ... 3.13.0-96-generic #143-Ubuntu SMP ... i686 i686 i686 GNU/Linux
Browsers: Chrome 53.0.2785.116 m & Firefox 49.0 on Windows 10.
