two small mod_usertrack changes (again)

I submitted this a while ago, but it never got commited.

This adds a CookieDomain directive, for setting the domain on
mod_usertrack cookies, and adds "in-cookie" and "out-cookie"
notes to the request depending on whether the user tracking
cookie is inbound or outbound.

The patch includes documentation.

Pretty harmless stuff, but I've found it useful.
