[Bug 122737] TOC disappears when right click-update index/table, because the headings are not recognized as headings

I reproduced the described behavior on Windows 7 with AOO 4.0.0 and AOO 4.0.1.

Opening the test document in OOo 3.3.0, AOO 3.4.0 and AOO 3.4.1 results already
in an empty table of content.

For AOO 4.0.0 we had improved the import of the table of content for Microsoft
Word documents in binary format - see AOO 4.0.0 release notes. Before AOO 4.0.0
the current content of the TOC is not imported and the TOC is newly created
based on the given TOC template information. Since AOO 4.0.0 the current
content of the TOC is imported as it is.

The import of the given document does not import the headings correct - they
are not recognized as headings. Thus, if you update the TOC it will be empty.
