API_EXPORT(index_of_response) ?

This one is neither static nor exported at the moment, it's used by
mod_perl in a method called custom_response(), which works like
ErrorDocument, but is done at request time. It's proved quite valuable
to quite a few people, except the win32 folks I forgot about:

#ifdef WIN32
    croak("Apache->custom_response not supported under win32!");
#else

This method also requires core_module to be visible, but we've done that
already.
Any problems with exporting index_of_response() ?
