Requested Screen not found exception

Can anyone suggest a reason why turbine is unable to locate a screen after
an action is performed.

I have a form which posts information, an action deals with the request
successfully, then it should load the designated follow on screen.  For some
reason the screen cannot be found.  It is there and it does work when I
request it.  It only fails after I use this particular action (the action
seems to work correctly)?
