We've noticed a number of anomalies in our database caused by ill-defined forms and/or table schema definitions. This PR:
- Adds the potentially missing legacy constraint which was added in PR https://github.com/apache/incubator-superset/pull/3978 which didn't contain the corresponding migration.
- Ensures that the `column_name` be non-null.
