I've also confirmed that no attempt to deliver the new account message 
is being made.

Do any of us have access to the machine to look at why this is 
happening, or do we need to open a bug with infra?

I'm having the same problem.  I dropped Spamcop and tried to have it
email me my password/token and I don't receive the email.

     An account was created for the address you sent this from.  Is your
     mail
     server blocking mail from hermes.apache.org
     <http://hermes.apache.org> [140.211.11.2 <http://140.211.11.2>] 
which is
     reportedly listed by Spamcop again?
