Hi, I looked at this and found what causes this.

Remove the keep-together from the fourth block in the second cell as indicated in the image below and it will publish nicely (at least with FOP 2.2 that I have):

I don't know why this happens though but my experience is that keep-together can sometimes cause frustrating issues.
