doctype howto updated

i have updated and corrected trapo's doctype howto on the wiki.

i hope the additional information will be useful.

what i really liked about the document were the FIXME's which made it 
very easy for me to go in and add explanations where things were 
unclear. i encourage others to add FIXME's for concepts they do not 
understand.
