Did you mean to leave out the <env-entry-value> tag in your web.xml?

The web.xml DTD says it is optional, but that it must be supplied 
during deployment.  Not sure how that is supposed to happen, but
my guess is the web.xml one is incomplete and knocking out the
server.xml setting.
