Forbid AI attribution in commits and pull requests

The initial import commit carried a Co-Authored-By trailer naming an AI
tool. Apache project history records people, so CLAUDE.md now states
the rule plainly: no trailer, no "Generated with" line, no other
mention of an AI tool in a commit message or a pull request
    description.
