JIRA is often used by contributors who have not signed the CLA and not read the development process guidelines. They find a bug, they send a patch, they do not bother acquainting themselves with all the intricacies of the Apache process.  You want their express permission, so the contribute option should be off by default, otherwise, did they really mean it, or just didn't read the whole page through?

(And we do on occasion get attachments that are not for inclusion in the code, e.g. stack traces or screenshots)

But as a committer, I find it annoying if I have to go through approval each time I contribute something.  I signed the CLA, read all the relevant text, follow the discussions with interest, etc.  If I didn't mean to contribute something, and that hasn't happened so far, I'll say so explicitly.
