4.5.6 in the documentation says:
"Be sure to configure a load balancer for the Management Servers. See Management Server Load Balancing."

However, the documentation contains no section labelled as such. This section existed in the CloudStack 3 install guide:

And needs to be added back into the CloudStack 4 documentation.
