Form Widget Help

Hello Friends,

How can I create a Form Widget with type list for following HTML.

<table>
  <tr> 
    <td colspan="5"></td>
  </tr>
  <tr> 
    <td>A</td>
    <td>B</td>
    <td>C</td>
    <td>D</td>
    <td>E</td>
  </tr>
</table>
