sample doc with a ToC
This bug looks like 7734 (fixed in 2003). Apparently it reappeared in the
latest version of OOo.
The problem is the following: automatically generated Table of Contents (ToC)
does not maintain levels of ToC). When I open the document the number of levels
is always 1.
My system:
Version: OOO330m20 (Build 9567)
Dutch version, Mac

Both theory and introduction are shown in attachment.
But when right click - update index/table -> introduction disappears and only
theory is visible.
