patches to add attribute/elementFormDefault support for SchemaGrammar

Hello,

Here is a patch to add access to elementFormDefault and
attributeFormDefault.
I also fix what I think is a bug in XSDHandler, because dDoc2SystemId
seems to have Document objects as keys.
