[c++] Renaming session -> connection?

I'd like to do a big 'ole s/Session/Connection/ on the C++ code base to
make the terminology consistent with the spec. 

Related note: does svn really treat renames as remove/add as the docs
suggest? If so this change will cause much pain to anyone with private
work on the affected files so I'll co-ordinate a quiet time to do it.
