Some promotion scenarios aren't handled correctly by the builders

The builders don't handle some promotion scenarios correctly.  See TUSCANY-2324 and TUSCANY-2352 for descriptions of some of the problems.  In looking into this area and trying to fix TUSCANY-2324 and validate the fix for TUSCANY-2352, I found a few other things that did not appear to be correct.

In trying to understand what the problems are, it has become clear that we need a test framework specifically for the builders, so that the results of various different scenarios can be validated by introspecting the model without needing to start a runtime.

I have commited revision r678491 to the 1.3 branch to fix this problem.  The changes aren't yet in trunk.  I'll do that asap.
