Restore page metadata lost in the xdoc to Markdown migration
The five pages converted in #621 kept their metadata as a raw <head>
block, which the Markdown parser does not honor: the title and authors
were rendered as stray body text and the pages fell back to the bare
'Maven' title. Move the metadata to YAML front matter, the form the
parser supports. The scm.md <head> block itself is left for #1652,
which also fixes the unclosed <object> tags on that page.
