The format specified is exactly what you asked for - however, that is flawed because it should be: 
    subject-fmt = git commit: [%(repo_name)s] [%(branch)s] - %(subject)s

Notice the single character difference there? 

I've changed that in the git config, so it should be working now. Please let us know.
