[C1] FOP samples fixed

The 2nd and 3rd FOP samples have been fixed in CVS to conform with FOP 0.15
- belated thanks to Steve Coffman for that.

The 1st one generates warnings but still works! Steve actually submitted a
new version of it, but by mistake it turned out to be a copy of the second
file. Have you still got a FOP-0.15-compatible copy of test-fo.xml, Steve?

[Sod's Law: My browser crashes just as I'm writing the last email of the
day. Bah! :-]
