1) IgniteCacheDatabaseSharedManager.initializeForCache - dead code
2) IgniteCacheOffheapManager.mvccMaxVersion - dead code
3) IgniteCacheOffheapManagerImpl - TODOs
4) MvccProcessor.waitList - can't we use CHM here? Appears to be another scalability bottlneck in case of massive conflicting updates.
