fix(audit): list every failing operation of a repeated target

The repeated-failure-target table showed at most three of a target's failing operation types with no
indication that it was truncating, although the analysis derives the row's level from all of them
(`operationTypes.some(isHighRiskAuditOperation)`). A target flagged as a warning because one high-risk
operation failed could therefore be shown without that operation, leaving the flag unexplained.

The cell now lists every failing operation type; the tags already wrap. No user-facing copy changes.
