FORM for document-12?

I need to have to include a FORM in one of my documents. Rather than re-invent 
the wheel, I'd like to know if there's a FORM DTD and *.xsl that I can use -- 
does anyone have a set they'd be willing to share?

Right now there is no forms in Forrest. However, if you turn off 
validation for your xdocs (see forrest.properties) and use XHTML forms 
it will work fine.
