This might make sense in the case of the default publication, but it 
doesn't have
to be true for any other publication and it's too restrictive with 
regard to the choice of URLs.

we agreed on well-understood terms for the various parts of a URI,
and adding yet another variable doesn't help.

if document-id doesn't provide the correct value for a publication then
either the publication needs to be fixed, or the document-id reexamined.

But for this puropse you can use the document-url page envelope
parameter, can't you?

One problem seems to be that the document-id is just determined by 
removing the suffix and the suffix is forgotten and the live URI cannot
be generated.
