Attachment stream?

Hi- does anyone know how to get attachments as streams when deploying webservices with Axis 1.4 using wsdd?  I have a service that takes a value object containing a DataHandler, but I want to access the stream directly without having Axis cache it.  I have read the http://issues.apache.org/jira/browse/AXIS-2221 issue but I can't figure out how to put it to use...
