and port that was used to serve the console into the console's connect form

Ernest Allen resolved DISPATCH-1612.

Fix Version/s: 1.12.0
Resolution: Fixed
Should now use the browser's address and port as the default if auto-connect fails and the user has never successfully connected from that address.
Also fixed was a problem displaying the connect form after the user manually disconnects. Now, instead of showing the spinning wheels forever, it should display the connect form.
