Trouble to PDFToImage and iText

I got problems rendering PDF created with iText with PDFToImage.

The problem consist that PDFToImage don't write the the added with iText.

You can reproduce the problem using iText examples in
http://itextdocs.lowagie.com/tutorial/general/index.php

Any ideas to solve the problem ? Is it  a bug ??
