20 months later...

Turns out we also needed to change our db connection URLs (in the 
datasources in cocoon.xconf).
We had to change 'jdbc:Microsoft:sqlserver://' to 'jdbc:sqlserver://', 
i.e. get rid of Microsoft and everything was fine. ;-)

Thanks to Rick Harman for discovering this.
