Karl Pauls resolved FELIX-5797.

    Resolution: Fixed

Done in r1826988. 

Turned out we had some "bug" where we didn't remove refresh/uninstalled extensions from the system bundle. I fixed that along the way. 

Furthermore, we now will stop with a "needs new classloader" status code if an extension bundle is refreshed (as required by the spec).
