Somewhere there is a change from 4.0.x to 4.1.18 in the default web.xml that 
turns off the default invoker.

It took me several hours to flail around until I found the cause.  I saw no 
help on Bugzilla or on google so this is my contribution to save others time.

Solution: Uncomment the invoker section in the default web.xml.
