See spec http://www.w3.org/TR/xsl/slice6.html#fo_page-number
and http://www.dpawson.co.uk/xsl/sect3/bk/ch11.html#d0e8507

Look up
http://www.w3.org/TR/xsl/slice6.html#fo_page-number-citation
and the generate-id() function:
http://www.w3.org/TR/xslt#misc-func
The XSLT spec has an example for generating HTML hyperlinks
in section 12.2 which is very similar to generating an index.
As for your next question: no, you can't suppress repeated
page numbers due to the indexed stuff appearing multiple times
on the asme page.
